The following practice aids relate disability planning. Some of these aids are available only as html files, others are available only for download in WordPerfect format, and some are available in both formats.

Statutory Durable Power of Attorney Form	Power of attorney for property form adopted by the Texas Legislature for use on and after September 1, 1997. Form was not changed in 1999. Form is optional -- nonstandard form may be used.	WP Download
Modified Statutory Durable Power of Attorney Form With Disclosure	Glenn Karisch's modification of the statutory durable power of attorney form (with option for broader gift-giving powers, power to create trusts, and option for revocation by recordation), together with disclosure to client about the power of attorney	WP Download
Medical Power of Attorney Form	New form and disclosure statement adopted by Texas Legislature in 1999. Document must be "substantially" in this new form beginning with documents signed on or after September 1, 1999.	WP Download
Directive to Physicians and Family or Surrogates Form	New form adopted by the Texas Legislature for use on and after September 1, 1999. Form is optional -- nonstandard form may be used.	WP Download
Declaration for Mental Health Treatment Form	The Texas Legislature has adopted this form for use on and after September 1, 1997. Form was not changed in 1999. Form and disclosure statement are mandatory -- nonstandard form may not be used.	WP Download
Declaration of Guardian (Minor Children) Form	Texas Legislature modified this form in 1999 for use on and after September 1, 1999. Form is optional -- nonstandard form may be used.	WP Download
